Baker Chiropractic and Acupuncture offers our patient form(s) online so they can be completed it in the convenience of your own home or office.

• If you do not already have AdobeReader® installed on your computer, Click Here to download.
• Download the first three forms (Confidential Patient Information, HIPAA, and Consent to Treat), print them out and fill in the required information.
• Download the Motor Vehicle Accident Info page only if you are seeking treatment for a Motor Vehicle Accident.
  
• Please bring your completed forms to your appointment.  This will ensure that Dr. Baker can spend your entire appointment focusing on your care.
• If for any reason you cannot fill the forms out ahead of time, please arrive 15 minutes early to complete them.
