Baker Chiropractic and Acupuncture, LLC offers our patient form(s) online so they can be completed in the convenience of your own home or office.

Please carefully read the following instructions to ensure that you get the full amount of time for your new patient appointment and first treatment:

• If for any reason you cannot fill the forms out ahead of time, please arrive 30 minutes early to complete them.  


• Your first visit including Consultation, Exam, and Treatment is usually 90 minutes in length if you choose to receive either Chiropractic or Acupuncture.  If you have requested both Chiropractic & Acupuncture 2 hours and 15 minutes has been set aside for your appointment.  


• If you do not complete your New Patient Paperwork ahead of the starting time for your appointment, we may have to reschedule your appointment. 


• If you need to cancel or reschedule, please call or message us on our website at least 24 hours in advance to kindly allow another patient in need to receive care.  We often have a list of patients that need an appointment if a cancellation occurs.
